YOUNG LIFE CAMP

Each child must have a health form filled out for camp.

Follow the steps below to fill out health forms (You may save your work and return)

1. Go to healthform.younglife.org (or click the "Forms" button below)

2. Create an account using the Don’t Have an Account Yet link.

3. Click the Create New Form button.

4. Create your online signature.

5. Complete each page of the form, then click the Submit Form button.

You will need the following information to complete the form online:

· ​Camp name – Lake Champion ​· Camp dates – June 6, 2020 to June 12, 2020 · ​Home area – OH21
